Client Intake - Please Complete All Four(4) Sections Below AFTER Paying

Yes, 4 forms, but they're necessary and set us up for everything! Simply: a waiver (legally necessary), a client agreement so you and I are on the same page, a questionnaire about your environment and exercise preferences so that I can be prepared to maximize your paid time, and then a health questionnaire that's necessary so I know you're medically all clear to get after it.

I, the undersigned participant, acknowledge that I have knowingly, willingly and voluntarily chosen to participate in personal training services provided by Makeshift Fitness (Makeshift Fitness, LLC), including in-home and alternative training environments and potentially using non-workout-specific objects for exercise purposes. I understand that physical exercise and training involve inherent risks, including but not limited to muscle strains, sprains, injuries, or other health complications. I hereby release, waive, discharge, indemnify and hold harmless the trainer, Makeshift Fitness (Makeshift Fitness, LLC), and its agents from any and all liability, claims, demands, or causes of action that may arise from my participation in these services, including any injury or damage that may occur during or after the training sessions.

I acknowledge that I am aware of the risks involved in physical exercise and personal training. I certify that I am physically fit and have no medical conditions that would prevent me from safely participating in the training sessions. I assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.

STEP 2 - Client Training Agreement

Client Training Agreement

This Agreement is separate from the Waiver and Release of Liability and is intended to establish expectations and standards for the training relationship.

Section 1: Training Relationship & Commitment

Personal training is a collaborative process. Results depend on consistency, effort, communication, and mutual respect.

Section 2: Scheduling, Cancellation & Rescheduling

Sessions must be canceled or rescheduled at least 24 hours in advance. Late cancellations or no-shows may be forfeited unless otherwise discussed.

Section 3: Training Environment Standards

Training may take place in homes, gyms, outdoor spaces, or alternative environments. Clients agree to maintain a clear, safe space and appropriate personal hygiene.

Section 4: Flexibility & Creative Training

Makeshift Fitness embraces adaptability. Progress does not require perfect conditions - just commitment.

Section 5: Effort, Growth & Health Prioritization

Meaningful results require accountability, effort, and consistency.

Section 6: Communication & Respect

Open communication and professionalism are essential to a productive training relationship.

Section 7: Confidentiality (Trainer Commitment)

Makeshift Fitness agrees to strict confidentiality. No client information will be shared without explicit consent.

Section 8: Cost & Payment Expectations

The cost of services are:

  • $100 for (1) "Get Yourself Started" Pack, OR

  • $125 for (1) "Get Yourself Started" Pack + Assessments (e.g., Static posture; Overhead Squat)

  • $55 per 1 session of roughly 1 hour (must complete 1 of the "Get Yourself Started" packs to be eligible for single sessions)
